The Benefits of Walking – a summary of recent research from NCOR

Assessing the health benefits of outdoor walking groups

New health interventions advise us to become more active and walking groups are an increasingly popular means of activity.  This systematic review and meta-analysis looked at differences in physiological, psychological, and well-being outcomes.   A total of 42 studies involving 1843 participants were examined.

Statistically significant reductions were found in the mean differences for systolic blood pressure, diastolic blood pressure, resting heart rate, body fat, BMI, total cholesterol, and depression score.  An increase in VO2max was identified.  Evidence was less clear for changes in waist circumferences, serum lipids, and fasting lipids.

The Rugby Osteopath: Staying in the Game & Out of the Stands

We have seen some incredible displays of human athleticism, combining strength, speed and intuition in the rugby season so far. Unfortunately, it is also the time of year that rugby players, both professional and amateur, start trickling in to the clinic with various injuries that are associated with this high impact, high intensity sport.

The five most common injuries as identified by the RFU are:

5. Medial Collateral Ligament (MCL) Injury

What is it? The MCL is the ligament on the inside of the knee, which stabilises the knee during locomotion. Injury occurs when the fibres of the ligament are torn or ruptured.

Why injury occurs? MCL injury is most often a result of being tackled or sudden changes in direction. Heavy impact while the foot is planted pushes the medial (inside) knee joint apart, while direction changes cause torsion through the knee and stress the ligament. For these reasons back position players are most often affected.

4. Calf Muscle injury

What is it? Most commonly a tear of the Gastrocnemius or Soleus muscles, often at the point where these muscles blend with the Achilles tendon (2-4 inches above the heal).

Why injury occurs? Numerous factors can lead to calf injury ranging from insufficient training to poor arch mechanics and simply having short, tight calves.

3. Thigh Haematoma

What is it? Haematomas are severe bruises resulting from local blood vessel damage, caused by direct blow/trauma to the affected area.

Why injury occurs? A result of high impact collisions. In a sport with such physicality the injuries are unavoidable/bound to happen.

2. Hamstring muscle injury

What is it? A strain or tear of the biceps femoris, semitendinosus or semimembranosus (the hamstring muscles).

Why injury occurs? Similar to the Calf, hamstring injury can result from many things ranging from inadequate training, to poor arch mechanics, limited recovery time between matches and simply having short, tight hamstrings.

1. Concussion

What is it? A sudden, short-lived loss of mental function. It is the most common and least serious type of brain injury

Why injury occurs? Resulting from a blow or other injury to the head. A consequence of the physical nature of rugby.

Long Term Injuries

The above are the most common injuries, however, the following injuries keep players off the pitch for longer periods:

 * Shoulder dislocation – a result of heavy impact that is often reoccurring due to weakening of the surrounding ligaments.

Lumbar disc and nerve root damage – a result of spinal compression

that affects players in forward positions.

Tibio/fibula fracture – most likely caused by a tackle at high speed.

– Knee ligament injuries (ACL, PCL, LCL) – due to the torsional forces that are forced through the knee when turning in studs, damage due to scrimmaging and being tackled.

Treatment

Unfortunately, there is not much we can do about concussion. However, a combination of hands on treatment with rehabilitation often works successfully with acute injuries as well as chronic injuries that are part of complex postural patterns that build up over a period of time. We look to address the underlying causes of injuries, decreasing your chances of future injury.

Quadriceps muscle strains are the most common cause for local pain at the front of the thigh. Pain can also be referred to the front of the thigh from the lower back, hip or sacro-iliac joints (SIJ) so these are also important considerations. Quadriceps muscle strains often occur during sprinting, jumping or kicking. Like all strains they can be of different severities, it may be a sudden injury during activity where there will be acute pain, localised tenderness and if severe – swelling and bruising. More minor strains may not even be noticed until the following day.

Similarly, pain at the back of the thigh can potentially also be referred pain from the lower back, SIJ or hip. However hamstring muscle strains are also very common. Sports involving sprinting and jumping have an high incidence of hamstring injuries. Symptoms are similar to quadriceps injuries. There are number of factors which have been found to predispose to hamstring injury such as reduced flexibility, strength, lumbo-pelvic stability, joint dysfunction, fatigue and training modalities.

This study investigated a range of transient risk factors for the onset of acute low back pain among a sample of 999 patients. A range of issues were identified with which osteopaths will be familiar from patients’ reports. Risk factors included being distracted during an activity or task, a manual task with an awkward posture, a manual task with a heavy load, a slip, trip or fall in the 2 hours prior to onset of the pain, and fatigue. The risk of developing back pain was reported to be greatest between 7-00am and 12-00noon.

Adductor muscle and hip flexor strains are a common cause of groin pain, these are the muscles which attach at the top of you thigh and around your pelvis. Injury to these is common in sports where there may be sudden changes of direction or repetitive kicking movements. Pain can be very acute and is usually well localised to the area. Recurrent adductor and hip flexor strains are common for a number of reasons, its therefore important to carefully analyse why someone developed the initial injury, this often involves looking at biomechanics and running technique.

Bursa in the hip can sometimes become inflamed (bursitis). Bursae are little fluid filled sacs which act as a cushion between tendons and bones. Hip bursae can become inflamed with repetitive hip movement or trauma to the area. Bursitis usually causes pain and localised swelling and tenderness.

The tissue around the hip joint called the labrum can become torn, this may be due extrinsic factors such as trauma to the hip, excessive loading or repetitive activity both active (dancing, martial arts) and passive (cycling, driving for long periods) and intrinsic factors such as poor postural tone and abnormalities in the hip joint. Pain is usually dull and ‘nagging’ and often accompanied by ‘clicking’ or ‘catching’ sensation in the hip.

After taking a case history from you, your Osteopath will perform a series of simple test to gain a complete picture of which structure/s have been injured and then provide appropriate treatment and management. Treatment usually consistis of manual therapy, advice and exercise rehabilitation.

Hip and groin pain can sometimes be a symptom of a serious underlying medial condition. All osteopaths are trained to screen for this as part of their examination and will refer you to a medical professional if necessary.

What makes joints click? New research explains the noise when joints click.

spineThe cause of joint cracking has been previously attributed to gas bubble presence and collapse within the joints (http://www.ncor.org.uk/wp-content/uploads/2012/12/Audible_sounds_sm.pdf).   In a new study at the University of Alberta real-time magnetic resonance imaging demonstrates that the mechanism of joint cracking is related to cavity formation rather than bubble collapse.

The University of Alberta’s Faculty of Rehabilitation Medicine have published further details on the experiment, including photographs showing the MRI set-up.

Osteopaths may use a very specific form of joint manipulation called an HVT (high velocity thrust) where you may hear a click or a pop. They are trained for 4 or 5 years in the art of using this technique and create the movement by building up a series of levers to make the impact at the joint as minimal as possible. An osteopath should always ask for your consent when using this technique and explain the benefits and risks involved. It is one of many techqniques that osteopaths use to improve joint movement and release muscle tension.

 

To an osteopath, for your body to work well, its structure must also work well. So osteopaths work to restore your body to a state of balance, where possible without the use of drugs or surgery. Osteopaths use touch, physical manipulation, stretching and massage to increase the mobility of joints, to relieve muscle tension, to enhance the blood and nerve supply to tissues, and to help your body’s own healing mechanisms. What is Pilates?

Pilates is often compared to yoga, but they actually have very different backgrounds. While yoga is a centuries-old practice with close links to spirituality, Pilates is a mind and body conditioning technique developed by Joseph Pilates in the 1920s. Classes feature exercises to improve core stability and encourage healthy posture. Various pieces of equipment such as stability balls, resistance bands, foam rollers and Pilates studio equipment, like Reformers, may be used.

“What we’re really trying to do is teach people how to move better,” says Lynne Robinson, co-founder and director of Body Control Pilates. “We have three fundamentals – the ABCs. One is correcting the alignment of the body (A), then working on your breathing (B) and centring (C), which is core stability.”

Core stability is essential for good running technique and to help avoid injury. “In terms of runners, sometimes a slight imbalance in the body will go on to create problems because it’s such a repetitive movement that you’re doing. Even a small adjustment in your posture or in your core stability can make a huge impact,” says Robinson.

What’s the difference between matwork Pilates and Reformer Pilates?

Both classes work the same muscles in the same way, but just with different challenges. Matwork takes place on the floor and can be done without any equipment or with small items to make it more challenging (such as balls, bands and rollers). Reformer classes use Universal Reformers (see one here). These are frame structures with springs and pulleys that provide resistance as exercises are carried out, and give a symmetrical grid to work in.

What are the benefits of each kind?

With Reformers, the closed chain environment provides a particularly effective workout.  “We can really work on correcting hip, knee and ankle alignment, and improving the tracking of the knee”, Robinson says. “Particularly useful for runners with knee problems, we can focus on the vastus medialis obliquus, one of your quadriceps. Even though runners often get strong quads, this stabilising muscle can be weak. There’s also a Reformer attachment called a jump board, which is great for improving your running action.”

In terms of matwork, Robinson says the main benefit is “you can do matwork anywhere, in particular before and after your run. Unless you live above a studio you won’t have access to the equipment 24/7.” Additionally, Pilates accessories are widely available to buy if you wish to supplement your routine.

“With the Reformer, you probably get slightly quicker results than you will do just doing the matwork, however realistically most runners are not going to have access to a Reformer before and after they run. So, what they’ve got to do is come up with a few simple exercises to do.”

We are based just off Northcote Road near the Co-Op Supermarket. Please walk down to the end of Croosh Alley and come into the gym entrance to the right of the courtyard. There is a Reception Desk in the studio where someone can help you.

If so each company differ in their payment options. You will need to discuss the process with your insurer before your appointment. You may need a GP referral, an authorisation number and to find out your excess fee.

Tali has worked with top athletes and professional rugby players. She assesses ergonomic issues and sport related training techniques and can prescribe exercises for rehabilitation. Her treatment includes osteopathy, sports massage and medical acupuncture.

Tali Rayner M.Ost

Tali Rayner graduated from the British School of Osteopathy obtaining a Masters Degree in Osteopathy. Since graduating she has also completed training in Acupuncture and Medical dry needling lead by Dr Anthony Campbell.

She is registered with the General Osteopathic Council as well as the British Osteopathic Association. She has worked in a number of clinical settings including working with a professional rugby and football team.

Tali is structural in her approach to osteopathic assessments and uses a variety of treatment techniques including medical dry needling in conjunction with osteopathic techniques.

As part of your assessment Tali will assess work related ergonomic issuesor sport related training techniques, which may be contributing and maintaining  injuries and pain. Tali will prescribe exercises as part of your rehabilitation and stabilisation of an injury or longer term postural complaint.
